Incoming Medical Review
Before you come to campus, we will request that you complete a number of medical and health history questionnaires and forms online. In addition, we ask that you send any important information related to past injuries or surgeries to your athletic trainer by fax or email. Physcial procedures once on campus
- Completion of all paperwork; medical and all insurance information
- Sickle Cell Testing
- Cardiac Screen (Electrocardiogram (EKG) & Echocardiogram)
- Urine Sample
- Completion of concussion baseline testing
- Physical examination
Tryout Athletes
- All appropriate medical forms need to be completed
- Current insurance information has to be provided
- Insurance must be valid coverage within Lubbock county
- TTU try-out waiver must be signed
- Sickle Cell test results must be provided prior to participation in any try out (a receipt for payment of sickle cell testing will not be accepted).
- A pre-participation physical completed within the past six months
